Impact of training programme on the knowledge of nurses regarding nursing practice standards

Abstract : to ensure quality patient care and enhance career development ,competency levels of nursing professionals need to be identified and strengthened so that high standers of practice are established and maintained .indian nursing council has developed practice standards in six areas required to provide quality care. Various workshops ,each of ten days duration were organized at national institute of nursing education ,PGIMER Chandigarh in the months from October to December 2004 to acquaint the nursing personnel with the knowledge of these standards. A total of 67 nursing personnel were trained .pre and post evaluation at the end of each workshop was carried out .there was a statistically significant difference in the mean knowledge scores between the pre and the post-test (p<0.001) .nursing teachers had maximum mean knowledge score before as well as after the training .the net increase in mean score was maximum in nursing sister grade I followed by assistant nursing superintendent .As per the professional qualification the participants with M.SC .nursing degree had maximum score before as well as after attending the workshop although the respondents with B.SC .nursing post basic degree had maximum net increase in scores followed by the participant possessing GNM degree. The respondents with 11-15 years of experience had maximum net increase in the knowledge scores.

Patients are becoming so aware of health issues that they have been described as`expert witness` to health care process.Quality is rapidly becoming a concern to both the consumers and the providers of the services .in health care,quality being demanded and Expected .the nursing care providers are judged by the quality of services they provide. They are accountable for safe ,competent,knowledgeable and etical practices .thus quality in nursing care is major issue at present .in order to provide quality care.nurses need to be knowledgeable of patients’ expectations as the patients are discriminating consumers .quality is influenced by the level of knowledge and expertise of the concerned person.1 whenever a service is performed for a client ,the key to the quality of that service are the knowledge n,skills and attitudes of the person who deliver it. The qualified and motivated nurses deliver excellent nursing care that optimally benefits the patients and the familes.2 the measurement of the individual nurses ’ performance or behavior cannot be analyzed in vacuum .specific standards of behavior or performance must be set that serve as a measuring point or guidelines .the standards serve to establish norms and states what level of performance is required to obtain a specific desired outcome .3 they indicate a level of competency ,give a direction /framework for nurses that will ensure quality care and shall improve patient outcome and satisfaction.Basic qualification is important for nurses (as for any other profession ) to have broader understanding of the field .with time ,one tends to forget the theory and keeps on acquiring skills in their day to day practical spheres.With time and maturity ,it becomes essentiasl to revise theory in view of practical ,real life learning as well as to know about the newer concepts researches and developments in various fields . nurses being the major force as care providers are required to render the quality care providers are required to render the quality care to the patients. Indian standards on six areas required to provide quality care. these are: professional responsibility and accountability ; nursing practice ; communication and interpersonal relations ;valuing human beings; management technique and professional advancement .the nurses need to be knowledgeable regarding these standards before they start practicing in the clinical settings . workshops were organized for the nursing personnel with the objective to acquaint them with thje knowledge of these standards so that they apply these while working with their patients.each workshop was of ten days duration .the workshops were financed by ministry of health and family welfare ,govt of india and were organized at national institute of nursing education ,PGIMER Chandigarh in the months from October to December 2004.
Materials and methods :Total of 67 nursing personnel with different designations as staff nurses ,nursing sisters grade I, assistant nursing superintendents and nursing teachers attended the workshops .seventeen lectures on various topics related to performance standards were delivered by nursing experts. Pre and post evaluation was conducted .the questions asked were regarding legal and ethical aspects of nursing ,documentation in nursing ,therapeutic environment and biomedical waste management ,communication process and technique ,patient teaching ,nursing process, management techniques ,disaster management ,current trends and issues in nursing ,staff development and research in nursing ,staff development and research in nursing . three times during each workshop the participants were placed in the clinical setting to practice the scientific and systematic care through nursing process. On the whole they were given 25 hours and 30 hours for theory and practical respectively during the 10 days in each workshop.
Results
Profile characteristics of the participants :The mean age of the participants 35.21±8.19 with range of 25 to 57 years. Majority (97%) were female .maximum of them 49 (73.1%) were staff nurses,ten (14.9%) were nursing sister grade I,six (9%) were nursing teachers and remaining 2 (3%) were assistant nursing superintendents. The participants were from the states of Punjab ,Haryana ,Himachal and the various hospitals of U.T. Chandigarh .as per their professional qualification maximum participants 51 (76.1%) were possessing diploma in nursing ,Twelve (17.9%) were graduates in nursing and three (4.5%) were having M.SC nursing training .one of the participants was with PH.D. degree. the participants were with varied years of experience .about one third of them i.e.21(31.3%) were having their experience between 6-10 years of experience and 14 (20.9%) had less than five years of experience. Mean knowledge scores during pre and post –test
The mean knowledge score of the participants before starting the workshop was 28.70± 7.27 with the range of 10-43 and it improved to 38.31± 7.26 with the range of 25-54 .there was a statistically significant difference between the pre and post –test scores.(t= 11.71 ,p < 0.001).(table 2).
